The deadline for filing your LoDI FICA refund is fast approaching. All members must have their application in and postmarked by April 5, 2007. There will be no exceptions.
If you did not receive your letter you must call the Office of Payroll Administration at 212.669.2333 to request a duplicate letter. Time is of the essence so don’t delay.
It has come to our attention that some widows have not received a letter for their late husband, most often because of a change of address.
This letter is going out to all our active and retired members who are eligible but if you know a widow who has not received a letter please let her know she has to follow these instructions in order to receive the refund.
MILITARY LEAVE -- The matter of those eligible LoDI employees who are currently serving on military leave is being worked on with the IRS. If these employees have left a power of attorney, then the consent form should be completed, signed and submitted by Thursday, April 5, 2007 by the holder of the power of attorney.
DISPUTED AMOUNTS -- It has come to our attention that some employees who are disputing the amount of LoDI wages shown on their forms (i.e., missing periods) are not returning the form until the dispute is resolved. Please note, failure to return the consent form on or before Thursday, April 5, 2007, will forever bar these employees from collecting ANY of the refund due them. Thus, even if the dispute has not yet been resolved, merely circle Y for Yes both for the years included on the form, and for those years that are not included but for which they believe that you are owed a refund. This will protect their right to a refund from the IRS should it ultimately be determined, after investigation, that they are entitled to a refund for these disputed periods.
Please remember that no extensions can or will be granted just because an employee disputes an amount listed. Please encourage your employees who have not already returned their consent forms to do so as soon as possible.
