Health Plan Rate Changes – Effective July 2004
Basic Plan Rates: Payroll deduction rates for most of the basic plans changed with your first full payroll check in July. However, certain rates are subject to change, which may result in adjustments to future paychecks.
Optional Rider Rates: New payroll deductions for Optional Rider rates also began with your first full payroll check in July. You may not select individual benefits in the Option Rider. For the GHI plans and HIP/HMO the Optional Rider is a package. However, if your union welfare fund provides benefits similar to some or all of those listed in the Optional Rider for your plan, those particular benefits will not be available through the Optional Rider. In these cases, payroll deductions will be reduced accordingly for the benefits provided by your union welfare fund.
Please review and compare all deductions on your first full payroll check for July with the rates associated to the handout.
